What to Expect
How often will I need to come to physical therapy?
This will be determined by your physical therapists when they evaluate you.
What should I wear for my therapy?
During your session your physical therapist will require access to all areas of your body. Please wear shorts, tank top and/or sports bra. If you are coming from work you can change clothes in the restroom before therapy starts. Please avoid applying lotions the day of your visit.
What insurance do you accept?
Anthem, Aetna, First Health, United Health Care, Medicare, Signature Care, Tricare North, Encore, Indiana Health Network, Choice Care, PHP, Three Rivers Preferred, Lutheran Preferred, Unicare, Secure Horizons, Multiplan, PHCS, auto insurance, workers compensation.
**We are not in network with Medicaid and Healthy Indiana Plan
I don't see my insurance listed as one you take, what should I do?
We will see patients that have out of network insurance such as Sagamore. We will call the insurance company to get your benefits before you schedule an appointment with our office. Insurance companies have different benefits for out of network providers. We will call the patient that day and make them aware of the benefits quoted, then the patient can determine if they would like to schedule an appointment.
Will I have the same co-pay as when I go to the doctor?
In some cases this may true, however you may have a certain percentage you must pay each visit. It all depends on your insurance benefits for Out Patient Physical Therapy.
Will I know my insurance benefits before my appointment? What if I need prior authorization?
Your insurance company will be contacted before your first visit with us, we will give you the quoted benefits before your appointment starts or when you check out. If prior authorization is needed our office will obtain that.

What if I have a high insurance deductible to meet or can't pay my co-pay each visit?
We will work with you to establish the best possible route of payment.
Does my referral/script from the doctor expire?
Yes, you have 30 days to use the referral/script. If it's over 30days old you will need to attain a new one from your doctor.
What's the difference between Massage Therapy and Physical Therapy?
Physical therapy, you must be referred by a licensed physician. Therapy is usually filed through your health, auto or workmen compensation insurance. You have schedule appointments each week. The therapist will send updates of your progress to the physician that referred you. After you have met your goals from therapy you will be discharged as a patient.
Massage therapy, you do not need a referral from a physician. You do not file massage therapy through any insurance companies, it is strictly self-pay. You can come as often or as little as you like. Your chart will remain open, you do not discharge massage patients.
What are your hours of operation?
Monday Tuesday Thursday 7am-6pm, Wednesday 9am-6pm, and Friday 8am-5pm.
**Depending on cancellations and our patient load we may open later or close earlier. Our office is closed on major holidays.
